Making an Appointment

First Appointment

Making an appointment begins with Registering as a New Patient in our electronic Patient Portal. Once you have registered and are able to login using the username and password you've selected, please click on "Questionnaires" in the right hand column to fill out the necessary forms. Fill out all the New Patient Forms under "Intake Forms" and "Policy & Consent Forms" and any others that you've been instructed to complete. Please select which practitioner you'd like to see at the top of the main "Medical History" questionnaire. If you don't know which practitioner you'd like to see, select "I do not know". We will review your paperwork and assign you to the practitioner most fit to address your specific health concerns, then call you promptly within 24 - 48 hours to schedule an appointment. Please upload into the Patient Portal or bring with you to your first appointment any previous labs and procedures, or we may request that you get some lab work done before coming in.

What to Expect at Your Initial Appointment

First appointments are typically an hour and fifteen to an hour and a half long for new patient consultations. During this time we thoroughly review your history and present circumstances to get to the root of what is ailing you. From here, we begin to outline an individualized, therapeutic plan for you and order any additional blood work or specialized functional lab testing needed. Expect attentive listening and lots of questions so we can help connect all the dots to create optimal wellness! Many will be recommended to make an hour-long appointment with our dietitian to review specialized dietary recommendations, gut repair and detoxification programs as needed.

First Follow-Up Appointments

Functional Medicine aims to treat each person individually depending on what their particular needs are. Because of this, both the treatment plan and the time between your first appointment and follow-up will vary. Often, we recommend to follow-up with our patients in 3-6 weeks, but sometimes we need to follow-up in as little as one. Follow-ups are usually 50 to 60 minutes and are spent reviewing any recent lab work, assessment of your progress and current symptoms, and modifying the course of treatment going forward.

Payment for Services

In order to provide you with the time and quality of service your health deserves, Carpathia Collaborative DOES NOT TAKE INSURANCE for our consultation services. Many times, however, you will be able to use your insurance for blood work, certain advanced labs and any medications that are prescribed. Often, you can also use your HSA/FSA account to pay for our services/supplements as your insurer allows. We cannot guarantee what insurance will cover, but know that we will try our best to accommodate your needs.

Dr. Christensen is considered out of network with your insurance company and we are NOT contracted with MEDICARE. Her patients will be provided with appropriate ICD-10 and CPT codes to allow filing for out-of-network benefits.

Ultimately, we advise you to check your benefits with your insurance company prior to visiting with us as we cannot guarantee reimbursement.
